As part of our Reopening plan, we need to resurvey our families to indicate their desire to have their student(s) return to campus under the Simultaneous/Concurrent Model that was discussed at last week’s Reopening Forum. Your responses to our survey will help guide us in preparing for any reopening that is allowed by our district.

If you have not already done so, please fill out the survey once for every PHS student in your household. The deadline to fill out the survey is Tuesday, November 10: Click here to fill out the Family Choice Survey

Failure to complete this form by November 10 will result in your student being assigned to Virtual Learning for Trimester 2.
