CITY OF SAN BRUNO - SERVICE RECOGNITION PROGRAM NOMINATION FORM
The City of San Bruno has established a program to permanently honor those individuals and organizations that have provided service to our community. In order to evaluate prospective honorees, it is necessary that the City of San Bruno gain information regarding the nature and scope of the nominees service to the community. This nomination from is to be completed by the organization(s) making the nomination on behalf of the nominee. Please refer to the recognition criteria to determine the eligibility of the prospective nominee.

Carolyn had been a member of the San Bruno Beautification task force since 1997. She also served on the San Bruno Culture and Arts Commission as a charter member since the inception in 2003. She previously served on the Citizen's Crime Prevention Committee for 25 years, including 4 as chairperson. In acknowledgement of her service, the City declared May 2, 2002 as Carolyn Livengood day.
Her service to Skyline College began in 1964 as No County chairperson of the first bond campaign to build the college. Her support continued as a member of the Skyline President's Council until her recent passing. She was the first recipient of the "Skyline Shines Award."
Describe how the nominee made significant contributions to the development of new programs, the enhancement of established programs or by providing new or improved facilities. Describe how their leadership made a difference,
Carolyn also served on the San Mateo County Committee on School District Organization and for 23 years worked to support the needs of students and families. She was instrumental in the annexation of Pacific Heights Middle School and Portola Elementary School from the Pacifica School District and San Mateo High School Districts in 1996.
How did the service of the nominee provide a positive impact for the people of San Bruno?
In 1989 she was named/pictured in "San Bruno People & Places" book by Darold Fredericks as a citizen who shows concern & effort.
She was inducted to the Women's Halt of Fame in 2004. In August 2015, she received the San Bruno Mayor's Award and in November 2016, she received the Patriot of the Year Award from the San Mateo County Veteran's Commission.
Why was the nominee viewed as an "appropriate role model" within the community?
Carolyn was President of the Avenue of Flags Committee, Last year she and the committee worked together to include hundreds of members of our community in the distribution of wreaths at the Golden Gate National Cemetery. The tradition continues to grow today!
Which programs or locations were nominee most closely connected with?
Being on the Beautification Task Force, she coordinated and spread the word about Operation Clean Sweep in May. This event beautifies our City and also brings hundreds of members of the community together. She is an icon in San Bruno and wasn't shy about approaching anyone and everyone to let them know about an event happening in San Bruno. Despite health issues she continued to put her community first and her volunteerism efforts remained at the forefront until her recent passing.

The tradition continues to grow today, along with the annual display of hundreds of flags at the cemetery each Veterans Day.

Carolyn was closely connected with every program she was connected with. Whether it was with the City's Beautification Task Force, where she submitted nominations for numerous properties for the annual Beautification Awards, or her tireless efforts in improving Golden Gate National Cemetery and organizing spectacular events for Memorial Day and Veterans Day honoring our military.

RESOLUTION NO. 2018 -_____
RESOLUTION HONORING CAROLYN LIVENGOOD AS RECIPIENT OF THE RECOGNITION OF EXTRAORDINARY SERVICE AWARD
WHEREAS, the City of San Bruno has established a Memorial and Recognition Program Policy to permanently honor those individuals and organizations who have provided extraordinary service to our community; and,
WHEREAS, the San Bruno Recognition Sculpture was created as an artistic expression of the City's commitment to its citizens and as the primary means to publicly honor them; and,
WHEREAS, the Rotary Club of San Bruno and the San Bruno Lions Club submitted a nomination of Recognition of Extraordinary Service for Carolyn Livengood; and,
WHEREAS, Ms. Livengood was recognized for her service to the Recycling Committee, later known as the Beautification Task Force, charter membership on the San Bruno Culture and Arts Commission, service as a 25-year member of the Crime Prevention Committee;
WHEREAS, Ms. Livengood was passionate in her support for Skyline College, having served as the chairperson of the first bond campaign to raise funding to build the college and her service on the Skyline College President's Council until her passing. Ms. Livengood fondly and proudly frequently spoke about having been a member of the first graduating class of Skyline College.
WHEREAS, Ms. Livengood enjoyed a long and successful career as a journalist and covered stories and events for the local newspapers including the San Bruno Herald, San Mateo Times, and the Daily Journal where she always promoted San Bruno events in her articles. She always attributed her journalistic career and success to her Skyline College education.
WHEREAS, perhaps one of Ms. Livengood's greatest passions was her tireless work on behalf of United States Veterans. She was President of the Avenue of Flags Committee and worked tirelessly for the Golden Gate National Cemetery. As part of the committee she lead the annual Veterans Day and Memorial Day programs, the Wreaths Across America program, raised awareness about the cemetery's historical significance, and raised funds for beautification projects. She was instrumental in leading the initiative to replace the cyclone along Sneath Lane and San Bruno Avenue surrounding the National Cemetery with new wrought iron fencing. She also served on the committee which successfully modified the name of Sneath Lane to added Honorary Veterans Way.
WHEREAS, while Ms. Livengood was not a member of either of the two organizations who nominated her for the award recognition, she nonetheless was a contributor to their success and outreach to the community. She was always present selling fundraising tickets at the San Bruno Lions Club Pancake Breakfast and Rotary Club of San Bruno football/cheer luncheon. She was always eager to lend a hand where she was needed.
WHEREAS, two members of the Culture and Arts Commission, Parks and Recreation Commission, and Planning Commission reviewed the nomination and recommend unanimously to the City Council that Carolyn Livengood be the recipient of the 2018 Recognition of Extraordinary Service Award;
NOW, THEREFORE, BE IT RESOLVED that the San Bruno City Council hereby adopts a resolution honoring Carolyn Livengood as recipient of the Recognition of Extraordinary Service Award.
