Police said Meeker was the operator of the Nissan and was found unresponsive with no pulse, prompting immediate CPR from officers at the scene.
"After a prolonged attempt at resuscitation by Seymour ambulance EMTs and Valley Emergency Medical Services paramedics, Meeker was ultimately pronounced deceased," wrote the SPD.
Police said the driver of the Charger was an 18-year-old female who was "conscious and alert and appeared to have sustained non-life-threatening injuries," police wrote.
The female was taken to the hospital via Seymour Ambulance, police said.
The SPD said the nearby Monroe Police Department reported to Seymour that the Nissan Rogue was "operating erratically in their town on Route 34."
"This call came into the Seymour PD at (3:27 p.m.) Officers were enroute to the area to check for the vehicle when the crash was reported," the SPD wrote.
The crash prompted Route 34 to close for hours, reopening Thursday at 8:25 p.m., police said.
The SPD said the cause of the accident is still under investigation, and the Connecticut State Police CARS Unit is aiding Seymour with the investigation.
