Susan Callahan, RN, Joins H&H Home Care as Community Liaison

Susan Callahan has joined H&H Home Care as a Community Liaison, focused on developing and nurturing relationships with community organizations related to senior care, including nursing and assisted living facilities, retirement communities, physician offices and hospitals throughout Connecticut’s Fairfield and lower New Haven counties. She interacts with social workers, physicians, case managers and discharge planners on a regular basis to offer immediate and near-term senior home care needs for their patients and clients.

As a registered nurse (RN), Ms. Callahan has more than 30 years of progressive experience in the nursing field, including critical care nursing at Stamford Healthcare System. Her professional experience also includes case management, healthcare coordination, clinical liaison and marketing roles. At Visiting Nurse and Hospice of Fairfield County in Norwalk, she was a case manager for 35 patients from admission to discharge. At Sunrise Senior Living in Stamford, she was a healthcare coordinator responsible for the health and safety of 90 assisted living residents. She also served as clinical liaison and marketer at Healthbridge Management in Westport, CT.
