It's Time For Kindergarten Registration in Madison

Registration begins in March, school officials said.

From Madison Public Schools: The Office of the Superintendent of Schools has announced the dates and requirements for registration for students entering kindergarten for the 2019-2020 school year. Registration will be held at the Madison Public Schools’ Central Office at 10 Campus Drive.

Registration will be held Monday, March 25, 2019 from 3:00 PM to 7:00 PM and Tuesday, March 26, 2019 from 9:00 AM to 3:00 PM. Parents or guardians unable to attend the scheduled registration dates may contact Tania Kosiewicz at 203-245-6307.

For registration forms, please visit our website at www.madison.k12.ct.us . Registration forms are also available at the Madison Public Schools’ Central Office at 10 Campus Drive between the hours of 8:00 AM and 4:30 PM, Monday through Friday.

Requirements for Registration:

• Students eligible to enter kindergarten in September 2019 must reach their fifth birthday by
December 31, 2019.
• It is not necessary for the child to be present at registration.
• Birth certificates (a copy is sufficient) must be presented at the time of registration as well as proof of residency (3 different utility bills or parent's driver's license are sufficient if the address matches the child's residence)
• Kindergarten physicals performed 12 to 15 months prior to entry into Kindergarten, are acceptable, provided they are completed on the three-page State Health Assessment form. This form is not required at time of registration but must be submitted to the school nurse at least one month before school begins.
• Immunization records must be reviewed by the school nurse before a student may attend school. Please inform the school nurse of any significant health concerns.
• Students entering from outside of the U.S. are required to have a physical exam by a provider licensed to practice in the United States. Including the TB risk assessment as part of this exam.

PLEASE NOTE: Parents electing not to register their age-eligible child will be asked to sign a
waiver to that effect.
