CT Home Care Program for Elders Provider Conference, Breakfast Program

- CT Home Care Program for Elders (CTHCPE) Update – DSS will provide an update on what is new in the CTHCPE.
- CMS CAHPS Home and Community Based Services Survey - An overview of the state’s implementation of this cross-disability survey of home and community-based service beneficiary’s experience receiving long-term services and supports.
- The Universal Assessment Tool – An overview of this new client assessment tool that will be utilized throughout the state’s Medicaid waiver programs and which will be implemented on July 1 for the CTHCPE clients.
- No Wrong Door & MyPlaceCT.org - An overview of Connecticut’s resources that help individuals live in the place they call home by making it easier to find everything from long-term care to services and supports.
- Hospice Utilization in CT – A review of a new study looking at the state’s underutilization of hospice services.
