A Conference for all Providers Participating in the CT Home Care Program for Elders on Tuesday, June 19, 2018 at Aqua Turf Club 556 Mulberry St, Plantsville

CT Home Care Program for Elders Provider Conference, Breakfast Program
8:30 – 9:00 a.m. ~ Registration and Hot Breakfast
9:00 to Noon ~ Presentation on the following topics:

  • CT Home Care Program for Elders (CTHCPE) Update – DSS will provide an update on what is new in the CTHCPE.
  • CMS CAHPS Home and Community Based Services Survey - An overview of the state’s implementation of this cross-disability survey of home and community-based service beneficiary’s experience receiving long-term services and supports.
  • The Universal Assessment Tool – An overview of this new client assessment tool that will be utilized throughout the state’s Medicaid waiver programs and which will be implemented on July 1 for the CTHCPE clients.
  • No Wrong Door & MyPlaceCT.org - An overview of Connecticut’s resources that help individuals live in the place they call home by making it easier to find everything from long-term care to services and supports.
  • Hospice Utilization in CT – A review of a new study looking at the state’s underutilization of hospice services.
