Participate in the Community Health Needs Assessment

Middlesex Health is conducting a community health needs assessment and we are hoping to get your help as key community stakeholders and engaged community members/representatives. This is a regional effort with local health departments and multiple community organizations/agencies as collaborative partners.

We are looking to measure the collective community voice through a key informant survey (link is below). There are 10 health and health related questions with multiple indicators. The survey should take about 10 minutes to complete. All aggregated and anonymous responses will be included in the community health needs assessment.

The end-date for completion is Friday, September 6th.

Our Cancer Center is also conducting an assessment and is looking for input on barriers to cancer screening, diagnosis/treatment, and survivorship care. The results will help guide Cancer Center planning. This survey should take about 5-15 minutes to complete, depending on whether you have been diagnosed with cancer, and the link is below. We appreciate you sharing this link with friends, colleagues, and those you serve, particularly if they are members of populations likely to encounter barriers to care.
