Live Well and Aging Mastery Programs And More

The State Department on Aging is pleased to announce the launch of www.cthealthyliving.org which provides easy access to learn about and sign up for Live Well and Aging Mastery Programs® across Connecticut.

Live Well is Connecticut’s suite of evidence-based self-management programs, developed by Stanford University which includes Live Well with Chronic Conditions, Live Well with Chronic Pain and Live Well with Diabetes (in English and Spanish). The Aging Mastery Program®, developed by the National Council on Aging, is a fun, innovative, and person-centered health and wellness education program that builds sustainable behaviors across many dimensions that lead to improved health, stronger economic security, enhanced well-being, and increased societal participation.

This online resource cthealthyliving.org was developed collaboratively with the Connecticut State Department on Aging, Department of Public Health, regional coordinators from the five Area Agencies on Aging, Connecticut Community Care and Qualidigm.

The website is a key initiative of the CT Healthy Living Collective to link and network evidence-based programs delivered in communities across the state. The CT Healthy Living Collective is a collaboration of State Departments, local agencies and organizations and philanthropy dedicated to healthy aging and healthy communities throughout our state, and to providing leadership and visibility for the challenges and opportunities faced by Connecticut’s growing aging population. The CT Healthy Living Collective is coordinated by Connecticut Community Care and made possible through grant funds from the Centers for Disease Control and Prevention.
