Nicholas Reppenhagen, 37, of Derby was arrested Nov. 21 after a vehicle crashed into a tree on New Lebbon Road.
His vehicle crossed into the opposite traffic lane and struck a true off the road, police said.

The reports came in Nov. 17 and the vehicle was stopped in the area of South Main Street and Pecks Lane.
Elizabeth Melega, 50, of Taunton, Mass. was charged with operating under the influence and failure to drive at a reasonable distance after she crashed into the rear of another vehicle on Nov. 19.
The accident occurred on Buttonball Drive.
