Report: Mystic Healthcare & Rehab Center​ Fined By State

Mystic Healthcare & Rehab Center​ was fined for a number of violations related to patients injuries, the Connecticut Health I-Team reported.

MYSTIC, CT—The state Department of Public Health has fined three nursing homes for various violations including Mystic Healthcare and Rehabilitation Center, according to a report from the Connecticut Health I-Team.

Mystic Healthcare and Rehabilitation Center was fined $7,340 for a number of violations, the C-HIT report noted.

In one case, a "resident with psychosis and anxiety had 14 unwitnessed falls between March 2018 and January 2019," C-HIT reported citing documents. The person was injured as a result of a number of those falls including a cut on the arm in a March 31, 2018 fall; leg bruises from a July 13 fall; and "a left femur fracture that required surgery Jan. 16, 2019."

In addition, C-HIT reported, a resident "developed a facility-acquired stage 2 pressure ulcer on the coccyx on Feb. 7, and an investigation found staff failed to take proper preventative measures." It was reported that according to DPH, in January and February, "43 shifts lacked documentation the resident had been repositioned and 28 shifts lacked documentation the resident was helped with mobility."

In another incident, on Nov. 24, 2018 a "resident’s leg got trapped under a Hoyer lift during a transfer."

It was reported by C-HIT that the "nurse aides helping the resident didn’t report the incident to nursing staff because the resident seemed fine, but the resident suffered a broken left hip." The report states that the incident should have been reported, the citation said.

Calls to the Mystic facility by C-HIT for comment were not returned.
