A resident with Alzheimer's disease was found dead outside the facility earlier this year, according to the Connecticut Health I-Team.

WALLINGFORD, CT — Skyview Rehab and Nursing of Wallingford was recently fined $10,000 by the state Department of Public Health after a resident with Alzheimer’s disease left the facility in January and was found dead about 50 feet from the facility, according to the Connecticut Health I-Team.
Cara Rosner of the Connecticut Health I-Team reports the resident was found wearing only a shirt and one sock, with bloody knees and muddy feet. An investigation found that staff “hadn’t performed required regular two-hour visual check-ins with residents,” according to Rosner.
