Remarks: Failure of CFSM/Person in Charge to ensure Food Safety regulations are being followed within facility. Employees changing gloves or putting on gloves after another task and not washing hands, and handling food with hands. Raw chicken stored above raw beef in walk-in cooler and above raw fish in walk-in freezer. Time controls not documented according to written procedure. Time for rice at sushi bar not documented according to written procedures. Time for rice placed on warmer container by employee. Potentially hazardous food held under Time as a Public Health Control shall be properly documented as according to written procedures. Lighter stored on shelf above food in prep cooler in kitchen. Three (3) containers of chafing dish fuel stored on shelf above food equipment. Container of butane fuel stored above containers of mayonnaise in sushi bar area. White tuna and white fish noted on previous routine inspection not corrected on menus to be honestly presented. Air gap required for plumbing fixtures filled in with spray foam at 3-compartment sink and ware-wash facility.

Particle board (for hot plates) and small wooden boards (hot plates) in disrepair, exposed wood. Ambient air of drawer cooler at 48 deg. F. Chicken, beef, tomatoes at 43 deg. F relocated walk in cooler. Ambient air of cooler must be at 41 deg. F or less and must maintain food at 41 deg. F or less.
