A Mall of Georgia restaurant failed a Gwinnett County health inspection this week, according to health department records.

Tin Lizzy Cantina, 3333 Buford Drive, scored a 65 (out of 100) on June 3, the report stated.

The restaurant previously scored 88 on Oct. 22, 2014 and 95 on Jan. 10, 2014.

Here’s a synopsis of some of the inspector’s observations:

  • Person in charge not performing duties of ensuring provisions of the Georgia Food Code are being followed and critical risk factors are being monitored in facility.
  • Employees changing/donning single-use gloves to handle food and not washing hands in between.
  • Person in Charge unaware of symptoms and illnesses associated with Employee Health policy.
  • No paper towels at hand sink at wait station across from bar and paper towel dispenser not working at hand sink in kitchen.
  • Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place.
  • Multiple chemical spray bottles unlabeled with chemical in bottle. Flies observed throughout facility.
