Observations: Person in charge not maintaining active managerial control over establishment. Bare hand contact with ready-to-eat foods. Mold like build up inside of dishwasher. Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place. Cooked potentially hazardous foods not cooled from 135F to 41F within a total of 6 hours. Facility using Food Saver machine to vacuum package raw beef. Vacuum packaged raw beef DISCARDED. Facility needs State approval and a State approved HACCP plan to utilize reduced oxygen packaging (ROP) on-site. Cooling method for soup on pot not effective. Employee cutting heads of lettuce prior to washing. Pots and other equipment stored on the floor under shelving. Walk in cooler leaking water from ceiling. 4 door cooler with ambient air at 54 deg. F and not holding food at 41 deg. F or less. Floors in walk in cooler rusted; in poor condition. Follow up within 10 days. If facility makes a U on the follow up or the next routine inspection, then the permit may be suspended.
