• Observed dish machine chlorine sanitizer not at proper minimum strength and so utensils not properly sanitized. CA: Replaced the empty container of sanitizer with a new bucket of sanitizer. cl 100ppm.
• Observed chicken thawed at room temperature. CA: Food must be thawed out under refrigeration unit that maintains 41 degrees or below or under running water. PIC submerged food under running water.

• Observed cutting board encrusted with soiled material. CA: PIC removed cutting board to be cleaned and sanitized.
• Observed chicken stew being reheated in a hot holding unit. CA: Reheating for hot holding shall be done rapidly in a microwave or stove top to a temperature of at least 165 degrees. PIC reheated the chicken stew in a microwave from 66 degrees to 170 degrees.
• Observed wall soiled with accumulated food debris. Observed wall soiled with accumulated black debris in dishwashing area. CA: Clean all kitchen walls.
• Observed buildup of mold-like substance on surface of nonfood-contact surface of cooler unit. CA: Clean and sanitize the cooler unit at all times.
• Observed leaking pipe at plumbing fixture of hand washing sink. CA: Leaking pipe must be repaired so the hand washing sink be accessible for use at all times. Fix by July 27.
