- Prep unit holding foods at 55°F. Tomatoes at 55°F discarded. Unit was adjusted. Do not store Time/Temp Control for Safety Foods (TCS) in unit until holding at 41°F or below.
- Gyro meat, previously cooked, on spindle at 99°F internally and 130°F on exterior. PIC stated it had been on spindle since 12 p.m. (inspection time 4 p.m.). Meat was discarded. No time system established. Hot TCS foods must be reheated to 165°F for hot holding and maintained at 135°F and above. Corrected On-Site.
- Observed screens propped open at serving windows. Service windows are designed to be self-closing and must be operated in that manner.

- CFSM not present during inspection.
- Certified Food Safety Manager does not demonstrate proficiency in active managerial control measures. CA: Persons in charge, managers or CFSM must be properly trained or retrained
- Employee beverages throughout kitchen. On tables, shelves, counter tops. Water bottles, cups, cups without lids and/or straws.
- Multiple dented cans found mixed in with stock supply on shelving.
- Insects seen crawling in bin of uncooked rice (cockroach infestation: multiple roaches seen throughout kitchen, on top of equipment, crawling in bin of rice, dead bodies on top shelf adjacent to cooking/serving counter by wires and monitor).
- Observed improper vertical separation of raw animal foods and ready-to-eat foods; raw chicken above raw fish in reach in cooler.
- Soda syrups expired: Hi-C, Coke Zero, Diet Coke
- Inconsistent date marking of foods throughout kitchen. Must establish consistent practice of date marking prepared foods.
- Mold smell/seen inside non-functioning walk-in unit that is being used as dry storage. Must be treated to prevent contamination of foods in unit.
- No bucket of sanitizing solution for wet wiping cloths; wiping towels scattered throughout kitchen, on shelves, counter tops, tables, etc.
- Food service permit not posted.
- Cups, bowls used as scoops for foods; scoop handles not stored above surface of food, laying flat.
- Food debris on shelving throughout kitchen, upper counter tops, and inside walk-in units. General (thorough) cleaning needed.
- No proper chemical test kit provided for measuring the concentration of the sanitizer solution used for wiping cloths. No chemical test kit provided when using chemical sanitizer at three-compartment sink/warewashing machine.
- Multiple gaskets broken throughout kitchen: reach-in drawers, walk-in freezer, etc.
- No hot water available for hand-washing sink in kitchen: handle non-functioning.
- Garbage can full of water adjacent to ice-machine; employee states that is runoff/condensation from the machine as it runs; improper disposal of waste-water -- must be piped to drain, not held in garbage can and siphoned off with submerged pump.
- A follow-up inspection will be held on Tuesday, June 14.
