Items stored in prep top cooler were discarded onsite. Follow-up must be scheduled to observe the operational status of prep-top cooler.
• Observed items stored in main kitchen walk-in cooler without a date marking. Fully cooked/ready-to-eat items that have been stored for over 24 hours must have either a "made date" or "discard-by date" label. Items was corrected on site.

• Observed failure by food service employees to properly wash their hands (hands being washed in sink with no hot water). Employees must properly wash hands with soap and water that reaches a minimum of 100 degrees F. Legal notice was issued stating that facility must designate sink with hot water as hand sink until original hand sink is in working order.
• Observed raw beef stored above ready-to-eat salad in main kitchen walk-in cooler (repeat). Food must be stored in the correct descending order.
• Observed raw beef stored above raw sliced onions. Must store foods in correct descending order.
• Observed dishes being washed in dish machine which is failing to dispense sanitizer/failure to properly wash dishes. All dishes must be washed, rinsed, and sanitized.
• Observed diced tomatoes stored in walk-in cooler with an internal temperature of 58 degrees F. Tomatoes were date marked as being prepped Wednesday, Aug. 9, giving them over 4 hours to cool to 41 degrees F or lower. Advised food be discarded.
• Observed failure to provide documentation outlining method with which pickling takes place. Unable to verify from person in charge whether pickling is done for flavor or preservation. Facility must provide evidence verifying that a variance is not needed for pickling method.
• Observed failure to have an internal thermometer in front food service prep top cooler.
• Observed containers of food stored on the floor of walk-in cooling units throughout facility. Everything must be stored at least 6 inches off the floor.
• Observed failure to have food service permit posted for public observation. Must have posted.
• Observed failure to have most recent food inspection posted for public observation/facility misleading the public. Must have recent health inspection posted.
• Observed shelving in main kitchen area lined with raw wood. All kitchen surfaces must be smooth, sealed and easily cleanable.
• Observed failure to have hot water at main kitchen hand sink available. Must have hot water readily available at all sinks.
• Observed several fruit flies in main kitchen area. Must treat.
• A follow-up inspection will be held on Aug. 24.

• Observed items stored in walk-in cooler left uncovered. All items that are stored must be covered so as to prevent potential contamination.
• Observed failure to consistently date mark items stored in walk-in cooler. All ready-to-eat items that have been stored for over 24 hours must have a label and date marking either indicating the "discard" date or "made" date.
• Observed containers of "in-process" raw chicken stored on the floor in main kitchen area. Everything must be stored at least 6 inches off the floor.
• Observed cans used for food storage in cold holding units throughout main kitchen area. When a canned food has been opened, the remaining food must be stored in a food grade container.
• Observed in-use knives stored in between equipment. All in-use utensils must be stored so as to prevent potential contamination. Item was corrected onsite.
• Observed build-up of debris on floors and equipment throughout main kitchen area. All non-food contact surfaces must be cleaned so as to prevent debris build up.
• Observed light in walk-in freezer in disrepair. Must be repaired.
• Observed door from kitchen leading to outside left opened. Doors in kitchen leading to outside must remain closed or be covered with a mesh enclosure so as to prevent the entry of pests.
• A follow-up inspection will be held Aug. 28.
