• Observed wiping cloths not stored in sanitizing solutions; 
• Observed debris inside the hood vents; 
• Observed wiping cloth covering a hole in the floor in the kitchen; repair the floor;
• Observed mop stored behind the mixer in the kitchen; 
• Observed debris under all of the equipment in the WIC in the kitchen.
