• Observed interior ceiling of ice machine lined with a black mold like substance. CA: Dump all ice out and detail clean.
• Observed that there were no sanitizing buckets prepared in the facility.
• Observed multiple potentially hazardous foods stored at temperatures over 41 degrees F. CA: Foods were moved to WIC, others (stored past 4 hrs) were discarded. Corrected On-Site.
• Observed prep unit cooler (farthest left) not able to hold foods at the correct temperature. Internal temp of 52 degrees F observed. CA: Do not use and fix immediately.
• Observed multiple wet wiping cloths located throughout the facility. CA: Store in saniziter buckets in between uses.
• Observed plastic container stored inside of chicken broth. CA: Container was discarded. Use a ladle instead.
• Observed plates not stored in a manner to prevent contamination. CA: Plates were inverted.
• A follow-up inspection will be held May 18.

• Observed raw chicken and raw beef at prep cooler 2 failing to maintain proper cold hold temperatures (CA): All TCS (temperature controlled for safety) food items for cold holding must be held at 41 F or below. (COS): Placed on ice.
• Observed rice and refried beans cooling from the night before holding above 41 F. (CA): Foods being cooled must be cooled from 135 F to 70 F within 2 hours and from 70 F to 41 F to within 4 hours for a total cooling time of 6 hours. (COS): Discarded.
• Observed several TCS food items in WIC not date marked. (CA): Food held longer than 24 hours must be date marked. (COS): Discarded.
• Observed prep cooler 2 in need of repair. (CA): Have cooler repaired; must maintain ambient air temperature of 41F or below.
• Observed food debris at bottom of prep coolers. (CA): All nonfood contact surfaces must be clean to sight and touch.
• Observed hood filters in need of cleaning. (CA): All physical facilities must be maintained in good repair.
• A follow-up inspection was scheduled, but no date was provided on the form.
