• Observed vegetables being prepped in 3-compartment sink. Vegetable prep taking place next to soiled dishes. Must take place in vegetable prep sink.
• Observed failure to have food covered in cold holding units throughout facility.
• Observed failure to sanitize dishes used to serve food.
• Observed food left to thaw at room temperature Corrected On-Site.
• Observed food left to thaw in a container of standing water. Thawing must take place in a cold holding unit or under cool running water.
• Observed food stored on the floor in containers in dry storage area and main kitchen area. Must be at least 6 inches off the floor.
• Observed in-use utensils stored between equipment at grill area and dish washing area. Must be stored so as to prevent contamination. Corrected On-Site.
• Observed dishes being stored at meat prep sink. Prep sinks can only be used for food prep.
• Observed raw wood being used in food storage areas in main kitchen. All kitchen surfaces must be smooth, sealed and easily cleanable.
• Observed dish washing machine failing to dispense sanitizer.
• Observed build up of debris on equipment throughout main kitchen area.
• Observed build up of debris on floors, walls, and ceilings throughout main kitchen area.
• Observed failure to have lighting in refrigerator unit in main kitchen area
• Remarks: Facility must undergo name change procedures.
• A follow-up inspection is planned for Oct. 28.
