CA: Items must be stored in the correct, descending order based on final cook temperatures (e.i. ready-to-eat items at the top then swim, walk, and fly in descending order).
• Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit in walk-in cooler, salad reach-in cooler, grill prep prep top cooler, and pizza prep reach-in cooler (see "temperature observations"). CA: Items without time to cool were discarded. Items with time to cool underwent rapid cooling procedures (ice bath) (cos). Also observed salad refrigerator cool to 42 degrees before exiting facility. Advised that units failing to maintain required food temperatures be serviced.
• Observed failure to have thermometer in salad reach-in cooler. CA: Must have thermometers in all holding units. Item was corrected onsite.
• Observed ambient air thermometer not located in the warmest/coolest part of the grill prep top cooler unit. CA: Must have a thermometer located in the warmest part of unit.
• The report does not indicate if a follow-up inspection will take place.

Advised PIC to rearrange the food storage order: black bean patty on top, raw beef patty in the middle, and raw turkey patty at the bottom rack. Provided PIC a copy of correct food storage.
• Observed failure to properly cold-hold TCS food items on ice bath at 41F or below (raw chicken at 58F and butter milk at 55F). Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. Educated PIC that the food containers have to be submerged in ice (four corners in contact with ice) in order to cold-hold food items. Advised PIC to discard the chicken and the buttermilk.
• Observed food stored on floor (pickles in bins on floor next to the grill station, flours in packages on floor next to the fryer, vegetables in boxes and bread on floor in the walk in freezer). Food shall be protected from contamination by storing the food At least 6 inches (15 cm) above the floor. Advised PIC to elevate the food items.
• Observed in-use wet wiping cloths stored on top of cutting boards and grill station. Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution. Advised PIC to put the wiping cloths inside a sanitizer solution bucket.
• Remarks: This is a compliant investigation/inspection. Facility provided food source receipt -from Sysco.

Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables and cooked ready-to-eat food. Advised PIC to re-arrange the food items, cooked ready-to-eat food stored above raw animal food, and raw seafood stored above raw beef.
• Observed failure to properly cold-hold TCS food items in the back salad prep top cooler (liquid eggs at 46 F and boiled eggs at 52 F). Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. Advised PIC to discard the boiled eggs (made yesterday) and put the liquid eggs in the walk in cooler (made this morning).
• Observed the salad prep top cooler/reach in cooler in poor repair (leaking and air temperature at 46F). Equipment shall be maintained in a state of repair and condition. Advised PIC to replace the whole unit and stop putting food inside the cooler.
• Remarks: date mark items stored in the reach in coolers; keep the back door closed at all time; store food at least 6 inches above the floor; put thermometer inside the glass door reach in cooler; and replace cutting boards.
