  • Observed several cartons of liquid eggs stored for use with an expiration date of 10/21/2016. CA: All food items must be discarded after their respective expiration dates.
  • Observed container with shredded spinach stored on top of spinach without a barrier in between. CA: Do not store containers directly on top of food.
  • Observed employee prepping food without a hair restrain. CA: All food prep employees must have a hair restraint when prepping food items.
  • Reach-in freezer not operational. CA: All equipment must be maintained in good repair.

  • Observed two sauces hot holding in steam unit at 115 and 121 degrees F respectively. CA: Sauces were reheated back up to 165 degrees F. Corrected On-Site.
  • Observed multiple foods (sauces and chicken) stored at room temperature for up to 4 hours without any pre-approved procedures and records for using Time as a PH control. CA: Provide the proper documentation for Time as 'PH control' procedures and record all 'IN' and 'OUT' times for each food product.
  • Observed multiple foods held past 24 hours not date marked. CA: Foods were discarded.
  • Observed two batches of rice cooling down that were stacked on top of the other and covered tightly with foil. CA: Instructed PIC of proper cooling procedures.
  • Observed multiple knives stored in crevices between the equipment. CA: Knives were pulled and placed on the line to be rewashed/sanitized. Store on prep table instead.
  • Observed metal pans and dishes stored right side up in clean dish area. CA: Always invert dishes to allow proper air-drying and do not stack directly on top of the other while wet.
  • Observed that there were no test strips on site. CA: Be sure to order new ones immediately.
  • Observed back splash soiled with food buildup and debris.
  • Observed light bulb out at vent hood. Please replace.
  • Observed light bulb with missing shield. Get a light shield.

  • Observed cove base and wall separating main kitchen and grill prep area in disrepair. Must repair.
  • Observed exterior door in main kitchen propped open. Corrected On-Site.
  • Observed fruit flies in main kitchen area. Most recent pest treatment (done by Orkin) took place October 19th, 2016 and went to treat German cockroaches and rodents. No treatment for fruit flies noted on statement. Must take measures to prevent the entry and propagation of insects and other pest.

  • Observed food service employees fail to wash, rinse, and sanitize food contact surfaces. Soiled dishes must be washed, rinsed, and sanitized and this must only take place in the 3-compartment sink or in a dish washing machine.
  • Observed failure to have date markings on foods held in cold holding units. All ready-to-eat, temperature controlled foods held for over 24 hours must be date marked.
  • Observed food stored on the floor in main kitchen area. Everything must be at least 6 inches off the floor.
  • Observed wet wiping cloths left on counters throughout kitchen. Wet wiping cloths must be placed in a sanitizing solution when not in use.
  • Observed build up of debris on equipment throughout main kitchen.
