• Observed failure by food service employees to wash hands after changing tasks, handling soiled dishes, and changing gloves. Hand washing must take place between tasks and in between glove use.
• Observed failure to wash, rinse, and then sanitize food contact equipment and utensils in 3-compartment sink. Must wash, rinse, and sanitize all food contact surfaces and then you must allow them to air dry.
• Observed improper cooling time/temperature for cooked beef. PIC stated that cooling process for beef started at 10:30 a.m. Temperature taken at 12:50 p.m. was 75 degrees F. Food has 2 hours to cool from 135 degrees to 70 degrees F and then 4 hours to go from 70 degrees to 41 degrees F or lower. Advised that food be discarded/next time enact rapid cooling measures. Do not cling wrap foods that are cooling in cold holding unit - loosely cover so as to allow heat to escape. Corrected On-Site.
• Observed failure to have logs maintained for items held for 4 hours or less. Observed written statement that sushi items only held for 4 hours but facility failure to provide records for their use of time as a public health control.
• Observed frozen fish left to thaw at room temperature. Thawing must take place under cool running water or in a cold holding unit. Corrected On-Site
• Observed failure to provide variance documentation for sushi rice making procedure. If vinegar is used, facility must maintain logs and documents outlining the procedure.

• Observed cardboard used to line floor and shelving throughout main kitchen area. All kitchen surfaces must be smooth, sealed, and easily cleanable.
• Observed failure to have test strips for Quat sanitizer. Make sure that concentration is between 200 and 400 parts per million.
• Education provided for the following observations: Make sure to label dry food storage containers; Bowls may not be used as scoop; Must use scoop with handle and if stored in container the handle cannot touch the food; Hot holding must be 135 degree F or higher. Keep an eye on steamtable.
