• Observed boxes of "coating" stored in direct contact with the floor next to the chicken prep area. Food must be protected from contamination by storing the food in a clean, dry location; where it is not exposed to splash, dust, or other contamination; and at least 6 inches above the floor. Employee stored boxed containers of "coating" above chicken prep unit.
• Observed the handle of a dispensing utensil, within a bin of sugar, in direct contact with sugar in the main kitchen. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in food that is not a time / temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar or flour. Advised facility to remove dispensing utensil and replace with another dispensing utensils keeping the handle away from food.
• Observed boxed to-go containers stored in direct contact with the floor within the main kitchen underneath the mixer. Utensils, single-service and single-use articles must be protected from contamination by storing the such items in a clean, dry location; where it is not exposed to splash, dust, or other contamination; and at least 6 inches above the floor.
• Observed dirty walls adjacent to the 3-compartments sink, within the main kitchen, that was heavily soiled with a black residue. All physical facilities must be maintained in good repair and must be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to increase the cleaning frequency of the walls and floor within the warewashing area of the main kitchen.
• Observed walls behind the soda dispensing unit holding area within the rear of the main kitchen soiled with a black residue. All physical facilities must be maintained in good repair and must be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to increase the cleaning frequency of the walls and floor within the warewashing area of the main kitchen.
• Observed many dead insects trapped within the light fixtures throughout the main kitchen. Dead insects, rodents, and other pests must be removed from the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

• Observed uncovered containers of salsa within the reach in cooler located within the front food service area of the facility. Food must be covered and therefore protected from cross contamination while in storage. Advised facility manager to provide a covering for the containers of salsa.
• Observed uncovered containers of soup being hot held without a covering within the main kitchen. Food must be covered and therefore protected from cross contamination while in storage. Advised the facility manager to provide a covering for the containers of soup.
• Observed an accumulation of black residue inside the ice bin located within the main kitchen. Equipment food-contact surfaces and utensils must be clean to sight and touch. Advised facility manager to monitor and clean ice bin frequently to avoid the buildup of residue.
• A legal notice was issued advising the facility to obtain a permit that is current and not past its annual fee due date within 24 hours. Fiscal Services information was left with the facility for food service permit inquiries and payment.
• Observed flooring in the main kitchen heavily stained with an unfinished hard surface. Floor coverings, walls, wall coverings, and ceilings must be designed, constructed, and installed so they are smooth and easily cleanable. Advised PIC to make the necessary repairs to flooring in the main kitchen.
• Observed heavily soiled ceiling tiles above the ice machine located within the main kitchen of the facility. All physical facilities must be maintained in good repair. Advised PIC to make the necessary replacement of the ceiling tiles that are in disrepair.
