Observations: Handwashing sink by pizza station blocked with table. Paper towels not provided at two handwashing sinks. Interior of ice machine with slime like build up (around ice shoot). Towel in sanitizer bucket used to clean food contact surfaces with no QUAT detected in sanitizer. Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place. Hot-held potentially hazardous foods not maintained above 135F; no time controls/documentation in place. Ground beef did not cool to 41 deg. F within 4 hours after preparing. Cooked chicken cooling 3 hours at 80 deg. F discarded. Food prepared at room temperature must be cooled from room temperature to 41 deg. F within 4 hrs. Chicken covered in large bowl cooling for 3 hours at 80 deg. F. Raw beef cooling in covered container for 4 hours in prep cooler. Both items discarded.
