Remarks: There was failure on the part of management to address multiple risk factors in today's operation. Cross-contamination: Employee touched ready-to-eat sandwich buns and clean plates with gloves which had just contacted raw steak. Several wares had food residue and debris remaining after they had been washed and stored as clean, ready-to-use: 2 quart food containers, plates, beverage glasses, and coffee mugs. White colored filmy residue was left inside the beverage glasses. (third consecutive violation). Several food items were not being maintained at 41 F or lower (third consecutive violation). Portions of spaghetti squash have Tuesday labels placed over the original Sunday labels. Reach-in cooler in the bar was not provided air at 41 F or lower. Food permit suspended for three consecutive repeat violations
