Ice shoot of soda machine with heavy slime like build up. Written procedures for time as a control not available. Packaged food stored less than 6 inches off the floor (shelf with food hugs floor). Food must be stored 6 inches off the floor. Employee wearing jewelry other than a plain ring while preparing food. Bucket of sanitizer stored on floor.
