Remarks: Employee chewing gum in the prep area while handling food. Open can of Red Bull energy drink was stored on the prep counter. The gum and beverage were discarded. Cooked sirloin was not cooled using proper method (uneven cooling). Handle in contact with food item: Tongs of inadequate length were stored inside a container of lemon wedges.

Remarks: There was not active managerial control in place to ensure that all aspects of food safety are monitored routinely. Employee did not wash hands before donning gloves to continue working with food. Manager did not convey adequate knowledge of employee health policy requirements: symptoms, illnesses, and illness reporting requirements. Employee was chewing gum while preparing beverages in the bar. Parasite destruction letter was not on file for the salmon which may be served in a raw or undercooked state. Ice cubes were adulterated by dark colored residue dripping from the ceiling of the ice machine. Raw fish and seafood were stored underneath ground beef patties in the walk-in cooler. Unwashed produce was stored above ready-to-eat foods in the walk-in cooler. Employee plated ready-to-eat chicken salad panini on the raw fish breading station. he ice machine had thick layers of mold-like or yeast-like residues throughout its interior surfaces. Multiple foods were held in use past their 7 day refrigerated shelf-life. Employees observed wearing watches while preparing food. The watches were removed. Cloths for wiping food-contact surfaces improperly kept. Observed employee using the vegetable sink as a dump sink while bussing tables. Employee continued working with gloves to prepare ready-to-eat foods, after they became contaminated.

Remarks: Canadian bacon was held in use past its expiration date, 10/12/2014; an open package was in the reach-in cooler and unopened packages were inside the walk-in cooler. Green shelving inside the walk-in cooler has accumulation of mildew and mold-like residues.

Remarks: Foods inside the walk-in cooler were not maintained at 41 F or less. Ambient air temperature was at 44F. Freon leak caused refrigeration unit of the walk-in cooler to be unable to provide ambient air temperature of 41 F or less. Technician arrived during the inspection to recharge the freon gas. A repair part will not be installed until next Wednesday. However, the refrigeration repair company will send a technician to the facility on Monday to recharge it again.

Remarks: Employee was drinking from uncovered beverage while preparing food on the prep table. Employee placed bowl of noodles directly onto chunks of raw chicken left on the prep table after the chicken was cut. Employee touched the clean containers for soup with contaminated gloves. The employee had just cracked several raw eggs to prepare soup. A plastic bottle was cut in half and re-purposed as a chicken wing scoop. Discard the bottle. Multiple shelves in the kitchen area are lined with soiled, worn aluminum foil. Remove the foil from the shelves. Excessive food debris was accumulated underneath the slicer.
