Remarks: Salmon filets were cooked to 120 F and 140 F, respectively, then placed into service. The Hickory Bourbon Salmon entree does not bear a consumer advisory on the menus. Manager stated that the item is served fully cooked. Employee at grill removed the salmon from plates and cooked the salmon filets longer. Fish must be cooked to a final internal temperature of 145 F for at least 15 seconds (unless a proper consumer advisory and parasite destruction letter is in place). Employee personal items (such as cell phone and charger) improperly stored among restaurant items.

Remarks: Employee was observed washing hands by only rinsing them with water and using no soap, after handling raw meats. Sandwich buns and wheat tortillas are stored where exposed to splash from the handwashing sink. Accumulations of thin, whitish fungal growth resembling mildew are present on the food storage shelving inside the walk-in cooler. Excessive cardboard boxes were piled behind the receptacle. Dumpster doors were left open when not in use.
