Observations: Chlorine sanitizer registered too low. Chemical bottles unlabeled. Bottom of coolers with food residue. Stove top with residue. Exterior of microwave dirty. Food debris buildup on walls and splatter on cooler doors. Grease buildup on hoods. Observed multiple flies throughout facility.

Observations: Cheesecake and cookie dough bites were uncovered in the upright reach-in freezer. Chemicals were stored above sprinkles in the dry storage area. The vegetable sink was being used to wash equipment. A used glove was stored in the cheesecake bites.The facility uses quat ammonia for sanitizer. However, only chlorine test strips are present.Employee food (corn dogs) were stored above restaurant foods in the upright freezer.
