The restaurants were cited for issues such as dead and live roaches, black mold, raw food stored inside a shoe box and rotten produce.

- Observed failure to provide sanitize solution at concentration specified in 511-6-1 Food Service Code. Chlorine solution at 0 ppm. COS: Informed PIC to discard solution in all the buckets containing sanitize solution and make a sanitizer at proper concentration at compartment sink and refill buckets.
- Observed black and pink substance build up at the interior of the ice machine. COS: Informed PIC that the facility must increase the frequency of the detailed cleaning of the ice machine.
- Observed failure to properly cold hold TCS food products at 41 degrees Fahrenheit or below located in the prep top cooler at cook line. COS: Advised the PIC to discard the TCS items that were in the temperature danger zone
- Observed failure to properly hot hold a large container of rice on prep table in main kitchen. COS: Advised PIC to discard the rice.
- Observed failure to date mark ready to eat TCS foods stored in WIC. COS: Informed the PIC that the facility must mark the items with the date if the product is stored over 24 hours.
- Observed employee Claritin medicine and sanitizer tablets stored with food products on rack above compartment sink. COS: Advised PIC to remove the medicine from the preparation area, and to store sanitizer tablets away from food products.
- Observed bathroom cleaner in a spray bottle with out a label located under prep table. COS: Informed PIC that all cleaner must have a label, and advised the PIC to remove the bottle from preparation area until a label is provided.
- Observed failure to label food products with a common name once removed from original container or packaging. COS: Informed PIC that he must provide the common name of the food product on the container.
- Observed frozen raw shrimp sitting in a prep sink thawing; observed frozen vegetables sitting in a prep sink thawing. COS: Informed PIC of proper thawing methods that may be utilized in a food service facility.
- Observed several bags of partially frozen fries stored under counter at cook line. COS: Informed PIC of the proper thawing methods.
- A follow-up inspection will be held Nov. 11.

- Observed the facility removed employee restroom from the kitchen area. The facility must stay as permitted. A legal notice provided for the removal of the toilet and hand sink.

- Observed rotten lettuce and onions stored in RIC that is no longer properly operating. COS:PIC discarded the rotten food product.
- Observed raw chicken stored over ready to eat food products in the reach in freezer. COS: Informed PIC to remove chicken and place below all the other food products stored in the RIF.
- Observed several food containers with uncovered food stored in the RIC in kitchen. Observed several packages not properly closed in the RIF. COS: Informed PIC to protect and cover all food stored in refrigeration units.
- Observed failure to provide sanitize solution upon request. COS: Informed PIC that facility must make a sanitize solution at the beginning of service to utilize to frequently wipe down food contact surfaces.
- Observed heavy rust build up on can opener blade. COS: Informed PIC to clean can opener holster and change the blade.
- Observed failure to properly cold hold TCS food items stored in a prep top of a RIC that is in disrepair. COS: Advised PIC to discard the TCS products at 50 degrees Fahrenheit and store TCS food product in other RIC that is capable of keeping food product at 41 degrees Fahrenheit or below.
- Observed several dead roaches stored inside of RIC that is in disrepair, and light shields. Observed dead roaches around equipment throughout facility. COS: Informed PIC that the dead insects must be removed.
- Observed several live roaches and spiders during time of inspection. COS: Informed PIC that the facility must provide pest control from a licensed contractor within 10 days and provide invoice to health authority at follow up. Facility must obtain service from a licensed contractor at least once a month.

- Observed cook handling bell pepper with raw hands. Produce can not be touched with bare hands. Even if the facility plans to cook it the item may not be cooked to a temperature that will kill all contaminants. Cook advised that he can not cut with gloves as he needs to "feel" the items. This is not allowed. Please ensure that all produce and ready to eat items are not touched with bare hands.
- In WIC there is raw chicken above a container of margarine. In the WIF there are several items stored improperly. ALL raw meat must be stored BELOW cooked foods, ready to eat foods, produce etc. Observed in WIF raw fish above vegetables, raw meat above French fries, vegetables stored on raw meat. Rearrange the entire WIF so that all raw meat is below, and separated from, other food items.
- All cold food storage must remain 41 degrees or lower at all times even during the lunch rush. Observed meats on the line that were greater than 41 degrees and large amounts of meat stored in the WIC that were above 41 degrees. Consider making these units colder if constant opening is why the temperature is too high. If necessary consider using smaller food containers as this helps to facilitate colder temperatures.
- Almost every surface in the kitchen is dirty. Clean ALL equipment in the kitchen top to bottom and front to back. If it is in the kitchen then clean it inside and outside including handwash sink knobs, WIC fan covers, etc. Replace the WIC shelves as they are too rusty to properly clean and sanitize.
- The toilet room door must self close AND remain closed at all times.
- A follow-up inspection will be held Nov. 13.

- Observed several dented cans in the dry storage area in the main kitchen.Food shall be safe, unadulterated, and honestly presented. Advised and monitored PIC to discard all the dented cans.
- Observed raw food chicken stored over ready-to-eat food in the RIC in the main kitchen area. Food shall be protected from cross contamination by clearly separating cooked food from raw food. Advised PIC to separate foods.
- Observed chicken stored at a temperature above 41 degrees Fahrenheit in the main kitchen area next to food prep area. TCS food should be cold held at a temperature of 41 degrees Fahrenheit or below. Advised PIC to further cool the chicken.
- Observed food worker in the food preparation area without hair restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. Advised worker to wear hair restraint.
- Observed scoop submerged in bucket of flour. Scoops should be stored upright to prevent touching the food item. Advised PIC to ensure scoops are stored properly.
- Observed no container with a lid in the ladies restroom. Ladies restroom should have a container with a lid. Advised PIC to provide a container with a lid in the restroom.
- A follow-up inspection will be held Nov. 15.

- Observed several food items stored throughout the facility without a date mark to determine when to discard. All stored packages must have a date so that it can be determined when the food should be used or discarded.
- Found a container of shrimp dated 10/28/17 indicating that it has been in the facility 10 days. All hazardous food must be used or discarded after the 7th day. Shrimp was discarded.
- Observed fried rice cooked today in a container with an old date on it. All stored food must be dated with the correct date at the same time that it is stored.
- Observed food throughout the facility stored without a name on it to identify contents. All stored food must have a name on it.
- Do not use sponges in the facility. Sponges are prohibited and can not be used/reused to clean and sanitize. Scrubbing pads are allowed but sponges are not allowed.
- In order to use foil as a shelf liner it must be changed every day. In order to use cardboard as a liner it must be changed every day.

- Observed failure to properly date mark TCS ( time/ temperature control) stored in walk in cooler held longer than 24 hours. Except when packaging food using a reduced oxygen packaging method, and except as specified in paragraphs 4 and 5 of this subsection, refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1.
- Observed failure to provide up to date food service permit. Legal notice issued. Advised PIC to contact Fiscal Service in regards to payment information.
- Observed bowls being utilized to scoop dry goods. Must provide a scoop with handle and have handle stored above/ facing up right out of product.
- Observed failure to protect single service items stored at front food service. Must provide protective packaging or another means of protection from splash or dust.
- Observed reach in freezer near back dry storage area in disrepair. All equipment must be in good repair. Advised PIC to have freezer lid repaired.
- Observed failure to provide the correct sanitizer test strips upon request. Advised PIC to purchase carline sanitizer test strips if using chlorine in facility to clean and sanitize.

- Observed failure to provide paper towels at employee hand washing station near back ware wash area. Each hand sink shall be restocked with hand drying provision (disposable towels). COS. PIC restocked paper towels at employee hand washing sink
- Observed portable dispenser container stored on floor directly in front of employee hand sink. Employee hand sinks must be accessible at all times and not blocked by anything. COS. Item removed from directly in front of hand sink and off of floor by PIC.
- Observed pink like substance on the interior of the ice machine located near back ware wash area. All food contact surfaces must be clean to sight and touch. Advised PIC to drain ice out of ice machine and clean frequently and routinely to eliminate form of pink like substance.
- Observed in use utensil stored in sanitizer solution at the improper concentration. Advised PIC if using sanitizer water for in use utensil make sure sanitizer is between 200-400 (QUAT). COS. Replaced utensil and sanitized water.
- Observed failure to properly protect single service items at front food service line. Must provide protective packaging or another means of protection of cups from splash or dust.

- Observed lettuce prepped at ware washing compartment sink in kitchen.COS: Informed PIC to utilize the fruit and vegetable sink located next to compartment sink. Informed PIC that they may not prep the tilapia at that prep sink.
- Observed several portion cups utilized as scoop in seasoning and stored in containers. Observed scoop handles touching food product while stored in dry food bins. COS: Cups removed and handles of scoops placed upward and not in food product.
- Observed wet nesting in stacked containers. COS: Informed PIC that containers must completely dry before stacking and utilizing.
- Observed failure to utilized a commercial grade microwave in main kitchen. (Observed a white Sharp Carousel Microwave utilized) COS: Informed PIC once in disrepair facility must replace with a commercial grade microwave.
- Observed card board utilized to line all the shelves throughout facility and as a container to store squeeze bottles. COS: Informed PIC that all surfaces of a food preparation and food storage area must be smooth sealed and easily cleanable. Cardboard must be removed
- Observed food containers soiled with accumulated food debris. COS: Informed PIC that facility must increase frequency of cleaning the bins and containers utilize throughout facility.
- Observed failure to provide a covered trash receptacle in the women's restroom. COS: Informed PIC that a trash can with a lid must be provided for the disposal of sanitary napkins.
- A follow-up inspection will be held Nov. 12.

- Observed wet nesting of cleaned utensils next to the dish washing area.
- Observed ceiling tile missing in dish machine area. All physical facilities shall be maintained in good repair. Advised PIC to ensure the repair of the ceiling.
- Observed several live roaches in the main kitchen area. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: routinely inspecting incoming shipments of food and supplies; and routinely inspecting the premises for evidence of pests.
- A follow-up inspection will be held Nov. 11.

- Observed raw shell eggs that are not pasteurized pooled together improperly. In a HSP only pasteurized eggs may be pooled together. Otherwise, eggs must be cracked one at a time for immediate cooking and immediate service. Pooled eggs were discarded.
- Observed hot food piled high in containers and covered. Do not pile high and do not cover. Cool foods while stored in shallow pans and cover loosely such as with parchment paper. Using containers with lids will trap the heat inside and prevent the food item from cooling within the required time frame. Pureed chicken, mac and cheese meatloaf, greens observed.
- Until WIC is repaired do not store food in areas where the leak is as food can not be stored under potential water contamination.
- Observed WIC leaking from ceiling. Repair unit to prevent leak.
