Observations: Employee drinks found in unapproved containers (multi-use cups, drinks with no straws, drinks with no lids) throughout the facility. Drinks stored next to customer food. Drinks removed/discarded. Raw chicken being prepped on tables while vegetables, sauces and clean utensils were stored on the shelves below. Chickpeas, wontons cooling in deep, covered, plastic food containers in the upright cooler. Marinated chicken cooling in deep, covered, plastic food containers, stacked 2 containers high, in the walkin cooler. Had PIC uncover and unstack food to continue cooling. Raw shrimp thawing at room temperature in the kitchen. PIC had shrimp moved to the meat prep sink to continue thawing under cold, running water. A food dispensing utensil shall be available for each container displayed at a consumer self-service unit such as a buffet or salad bar. Rust-like buildup inside the ice machine. Broken handle on rice warmer.

Observations: Slicers, dicers, ladle, 2 knives stored as clean with food debris present. Employees wearing watch or wrist bands on arms. The employee removed the items. Utensils for self-service items too short for container. Utensils, Consumer Self-Service. A food dispensing utensil shall be available for each container displayed at a consumer self-service unit such as a buffet or salad bar. **The utensil’s length shall be longer than the widest portion of the container.**

Observations: Employee health policy was not in facility and person in charge was not able to decide which symptoms and illness must be reportable to the health department. Observed QUAT and a chemical named “Mixing Pot and Pan Detergent” being mixed together to clean all surfaces. Clean utensils being stored in a dirty container with food debris and a pair of keys.

Observations: Observed containers of (hazelnut, caramel, vanilla)sauces with no label. Observed containers being stacked while still wet and water debris at the bottom and sides of containers. Observed multiple clean lids with old sticker debris present. Observed hot water at the front hand sink at a temperature of 67F. Water was left running for 3 minutes and then rechecked and was at a temperature of 58F. A work order was put in during the inspection to get the temperature to 100F.
