Inspector Observations: Certified food safety manager certificate not posted at beginning of inspection. Person in charge unaware of the reportable health symptoms and/or reportable diseases. Opened employee water bottle stored with dry goods & opened can of coke (with straw) stored with clean equipment. Ice machine with build-up of black/pink mold like substances around food contact surfaces. Multiple individual servings of beef soup that were cooked on 09/28 were not date marked in the walk-in cooler. Fruit/vegetable sink was noted to have a cigarette butt, used cleaning sponge and a used scrubbing pad stored in the sink basin

Inspector Observations: Food handler donned new gloves for working with food without washing her hands. Cook handled raw shrimp skewers with gloved hands, changed gloves and then handled utensil to take pizza out of the oven; employee did not wash hands when switching tasks. Chicken roll-ups, beef roll-ups and chicken-spinach roll-ups holding <135F. Potentially hazardous foods not cooled to 41F within a total of 4 hours.
