A Cobb County day care which left a 2-year-old child inside a locked van has been ordered to cease operations after an investigation revealed systemic problems with the facility.

The New Beginning Learning Center on Sylvan Road was investigated by the state’s Department of Early Care and Learning after Jeremy Smith, Jr. was recovered from a locked van on Monday. Smith had been inside the van for two hours; day care employee Shavonda Dailey was arrested and charged for the incident.

State investigators uncovered a series of red flags with New Beginning’s travel and supervision practices, and determined the facility was too dangerous to remain open, WSB-TV reports.

The day care has appealed the closure order, which will allow it to remain open until a hearing into the allegations is complete.
