Yu Ka Roll and Pho received a 47/U grade on Sept. 15, 10 days following a 67/U on Sept. 5.
The restaurant was cited for multiple violations, including flies and bees landing on raw beef, food separation, holding and protection, and hand-washing.

Observations/remarks: Permit suspended for consecutive “U” scores. Violations for time as a public health control, food in good condition and unadulterated, using a handwashing sink, proper eating/drinking, separation of chemicals, food separation and protection (amongst other good retail practices) were observed today. Employee drink, in a multi-use cup, with no lid & no straw, stored on top of slicer, and above onions. Raw shrimp found inside hand sink by dish sink. Vegetable particles found in the hand sink near the fryer. Food removed from sinks, Observed flies and bees landing on open packages of beef which were stored on the ground outside of the food facility. Raw shrimp being prepped on table directly above partially covered sugar & salt, uncovered onions, and a blender. Soup holding on the stove measured 127F; the burner was turned off. Foods that use time as a public health control were not discarded after 4 hour time stamps. Spray bottle of windex stored next to food on the sushi bar shelves. Unlabeled containers of oils, sugar, salt, spices found throughout the facility. Multiple live flies found in kitchen.
