  • Observed encrusted food debris on stationary part of food slicer on back prep table. After using, all parts of slicer must be thoroughly cleaned and sanitized. Milk dispenser at shake station heavily coated with congealed and dried milk. This equipment should be cleaned routinely throughout the day to prevent this buildup.
  • Exteriors of equipment at shake station have thick food residue and are not being cleaned regularly. In addition, cooler interiors and door gaskets in main grill area have food buildup and must be cleaned and maintained.
  • There is an overall lack of adequate cleaning of floors and walls in kitchen areas. Thick, caked-on food and grease is visible underneath edges of equipment/tables. This is an on-going issue and must be addressed. Establish a cleaning schedule that includes monitoring that cleaning is being done. There is also stagnant, puddled water in several areas.
  • Observed live fly larvae in kitchen area floor next to ice machine. This is likely a result of the poor cleaning in facility. Clean and sanitize this area.
  • Remarks: Ensure signed agreements are available for employee health policy.
