• Verbal employee health policy does not include reportable symptoms/illnesses nor return to work guidelines. Also, there are no employee signatures on file. Website provided to obtain guidelines (also emailed handouts to Adler). All facilities must be aware of symptoms/illnesses requiring reporting and work exclusions/restrictions.
• No procedures for in-house cleanup of illness event. Website provided to obtain guidelines. All facilities must have disinfection procedures in case of illness event.
• Several working bottles of chemicals without content labels. Bottles were labeled on request. Corrected On-Site.
• Observed improper cooling procedure for cheese dip and broth. Very large containers of hot product sitting on top of ice will not cool properly. Ice water baths were prepared and products were placed in baths. Keep water level to product level when using this method (60 percent ice/40 percent water).
• Upright freezer is damaged on interior. Repair damage.
• Some equipment sitting on rocks and wooden boxes. These materials are not cleanable and cannot be used. Use proper equipment that is cleanable.
• Remarks: Need parasite letter for tilapia used for ceviche (product received frozen). Add "cooked to order" disclosure to menu.
