Reminder: Student Health Examination/Immunization forms are due by Friday, August 6, for the start of the 2021-22 school year.

These forms may be turned in to the WAC District Office ~ 1050 Ivy Hall Lane in Buffalo Grove ~ by July 30. 

In August, when the district's schools re-open, these forms may be turned in to your child's school.
