Providence Announces INTERACT™ Center of Excellence Designation

DOWNERS GROVE—Providence Healthcare & Rehabilitation Center is pleased to announce that it has achieved INTERACT™ (Intervention to Reduce Acute Care Transfers) Center of Excellence status.
To achieve this status, Providence committed to a rigorous certification program that promotes reducing unnecessary hospital readmissions utilizing the INTERACT™ Quality Improvement Program (QIP). This includes interventions designated to improve the identification, evaluation, and communication about changes in resident status. INTERACT was first designed in a project supported by the Centers for Medicare and Medicaid Services (CMS). Now, thousands of post-acute providers across the country are using INTERACT.

“As an INTERACT Center of Excellence, we are fully committed to providing the highest level of quality, utilizing the evidence-based Quality Improvement Program,” stated Megan Tengerstrom, Providence Life Services Vice President of Operations. “This positions our organization as a recognized leader with the knowledge and skills to reduce unnecessary hospital readmissions.”
In order to achieve this designation, Providence committed to an intensive focus on the following:

· Delivering leadership and direction for implementing full INTERACT QIP implementation.
· Identifying best practice approaches for utilization of the INTERACT QIP.
· Maintaining INTERACT™ Certified Champions.
· Monitoring data and clinical outcomes to identify areas for performance improvement.
· Providing ongoing training and support for the care delivery team.
