Have you considered types of health care treatments you would want to receive or maybe not receive in the event you quality of life was altered? Have you talked with your family about your health care wishes oftreatment?

Advance Directives may be the tool to facilitate this discussion. Only 15-20% of population has completed Advance Directive, consequently most people do not receive the care they want.   

Please join us for this program on Friday September 21, at 9:30 AM. A light breakfast is provided.   

The lecture is at the Good Samaritan Health and Wellness Center, Downers Grove, Rooms A & B (2^nd Level). The building is located in a separate complex north of the hospital. Please enter from the entrance on 31st street and proceed until the end of the lane.   

There is a nominal fee of $2.
