On August 30th, 2021, the Department of Health and Human Services (HHS), Health Resources and Services Administration (HRSA) recognized Greater Family Health with 6 National Awards! Greater Family Health’s clinical outcomes ranked in the top 15% of the Country’s 1,375 reporting Federally Qualified Health Centers, resulting in the receipt of the Health Center Quality Leader – Silver Award. Greater Family Health has received the National Health Center Quality Leader Award for 8 consecutive years, and the Advancing HIT for Quality Award every year it has been offered. Greater Family Health also remains a 2017 Million Hearts Hypertension Control Champion, a 5-year honor, awarded by the Centers for Disease Control and Prevention. We are incredibly proud of all our professionals that, together, have made this happen. We also want to thank our partners and stakeholders for their continuous support throughout the years. Their commitment to our organization has contributed to these accomplishments. It is inevitable that we will continue to age and, as we get older, things as simple as accessing health care can become challenging for us. At Greater Family Health, we believe that everyone should have access to the care they need and that our community deserves the best quality of care.  One of our priorities is addressing the barriers that our patients may encounter when seeking care. For seniors, these barriers include affordable care and medications, transportation for appointments, understanding and managing their care, and obtaining health insurance, to name a few. Our mission is to provide quality, affordable health care for all, including those without the ability to pay. We accept a range of Medicare plans Medicare plans , including those with dual eligibility. We offer information and assistance regarding applying for insurance benefits applying for insurance benefits , including health benefits for immigrant seniors. In addition, we offer a Sliding Fee Scale discount, which makes our care affordable.  According to the National Council on Aging, about 80% of aging adults develop at least one chronic disease. Diseases such as diabetes or arthritis may require expensive medications, but through the 340B Drug Discount Program, eligible patients can access discounts on their prescriptions. Furthermore, all our Health Centers offer transportation assistance and/or reimbursement which is very helpful for seniors that may struggle with transportation.    Managing medical, dental, behavioral health care and accessing specialists can get a bit confusing for anyone, but especially for older adults. Our Health Centers offer Integrated Care and Care Management services so that our patients’ experience is as smooth as possible. Our providers have access to a complete picture of our patients’ health across specialties which ensures a whole patient approach to our care. Our Care Management team helps our patients get the necessary referrals that work with their insurance and medical needs.  We are happy to help our patients navigate and access the health care they need.
