EPCHS Residency Verification Deadline Extended Through June 18
Evergreen Park Community High School asks parents and guardians to email proof of residency, or make an appointment June 16 or June 18.
EVERGREEN PARK, IL — Evergreen Park Community High School has extended the deadline to complete the residency verification process to Thursday, June 18. If possible, email the completed Residency Verification Form along with the supporting documents electronically. All EPCHS families are required to show provide proof of residency for the 2020-2021 school year.
Those who are unable to complete the process electronically, the high school will hold in-person residency verification assistance BY APPOINTMENT ONLY on Tuesday, June 16 and Thursday, June 18. Please call 708-398-1275 to make an appointment.

Email the proof of residency form and documentation to the EPCHS registrar at mbeglin@evergreeenpark.org.
Complete the residency verification form. All documents must be current and show parents/guardians’ name and address. PLEASE NOTE: If parent(s)/guardian(s) reside with someone else, the guardian(s) must provide documents from Categories A and C and the homeowner/lessor must provide documents from Categories A, B, and C, as well as fill out an Affidavit of Residency.

Category A (one of the following):
- State Issued Driver’s license
- State Issued ID
- Government issued photo ID
- Photo ID from foreign consulate
Category B (two of the following):
- Most recent property tax bill, including proof of payment (Owner)
- Current mortgage statement with proof of payment for property taxes (Owner)
- Homeowners insurance policy declaration page (Owner)
- Current lease AND proof of payment - Lease must be valid on the first day of the school year (Renter)
- Landlord’s Certificate of Inspection from the Village of Evergreen Park (Renter)
Category C (two of the following; must include parent name, address, and dated within the last 60 days):
- Current Gas Bill
- Current Electric Bill
- Current Water Bill
- Life Insurance Policy
- Medicaid Card
- Vehicle Registration
- Automobile Insurance Policy
- Payroll Stub
- Bank Statement
- Public Aid Card
