As we all take time to reflect on the past year and wait in anticipation of what is in store for us in 2014. I think back about 2013, of the countless number of families that Pam and I have been able to help and guide through times of stress and upheaval.
Just the other day – right before Christmas, I received a call from a woman and her daughter. Their husband / dad, was in the hospital, just out of Skilled Nursing with the prospect of returning to Skilled Nursing. They were overwhelmed about the options; what was the difference between Medicare and Medicaid; how do we protect her assets if dad goes on Medicaid; dad is a veteran – can that help; etc.
We had a good discussion. I was able to help educate them a bit on some of their options and connected them to a couple of resources, while coaching them with some tips for exploring Skilled Nursing Options. They were very appreciative of the time we spent and I will be following up to see “how things are going”.

This is what we do at Assisted Transition – our core business is helping families navigate the many options of Independent Living; Assisted Living; Memory and Private Duty Home Care. But along the way – we also provide a wide range of additional services and referral sources – depending on the needs and the situation. The best part of all of this is that it is FREE. No cost just to talk and see if we can help – that is what we are all about.
In the coming weeks, I will be sharing more stories and tell you more about my wife and myself. In the meantime – enjoy the holidays and Happy New Year!!!
