3 South Suburban Nursing Homes Fined

Nursing homes in Dolton, South Holland and Country Club Hills have been fined $25,000 by the Illinois Department of Public Health.

SPRINGFIELD, IL — Three nursing home and rehabilitation facilities in Chicago's south suburbs have been fined $25,000 by the Illinois Department of Public Health for violations officials say occurred during the third quarter of 2019. The state agency announced a number of fines and violations across the state on Friday, including ones at the Countryside Nursing & Rehab Center in Dolton, the South Holland Manor Health & Rehab in South Holland and Windsor Estates Nursing and Rehab in Country Club Hills.

All three were determined to have committed “A” violations, which according to the department of public health "pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result, or has resulted."

Countryside Nursing & Rehab Center, a 197-bed skilled and intermediate care facility at 1635 E. 154th St. in Dolton, was fined $25,000 for failure to prevent a cognitively impaired resident from being sexually coerced.

South Holland Manor Health & Rehab, a 216-bed skilled care facility at 2145 E. 170th St. in South Holland, was fined $25,000 for failure to follow its policy to have two staff members present when using a mechanical lift.

Windsor Estates Nursing and Rehab, a 200-bed skilled care facility at 18300 S. Lavergne in County Club Hills, was fined $25,000 for failure to supervise and implement interventions for a resident with a history of falls.
