Gun Found During Traffic Stop; Hit-And-Run DUI Driver: Blotter
Victim bloodied in fight outside restaurant; drunken motorist drives 30 feet off Route 41 : Lake Forest Police Blotter

LAKE FOREST, IL — The following information comes from Lake Forest and Lake Bluff police and Lake County court records as a record of those arrested by police. Readers are reminded that an arrest is not an indication of guilt, and criminal charges, which represent merely accusations by the state, are often dropped or reduced. Updated information may be available from the Lake County Circuit Clerk of Courts. All persons named are innocent unless proven guilty beyond a reasonable doubt in a court of law.
ARRESTS/CITATIONS
Steven Chuk, 32, of the 500 block of Everett Road, Lake Forest, was arrested by Winnetka police shortly after midnight on Sept. 21 inside Fitness Warehouse, 948 Green Bay Road, without the owners permission. Officers learned Chuk had two outstanding warrants on charges of burglary, theft and trespassing, issued in connection with Lake Forest police investigations of separate May and July incidents.
Steven J. Jones, 27, of the 2900 block of Brookside Avenue, Waukegan, was arrested around 12:30 a.m. on Sept. 26 and charged with unlawful use of a weapon and driving under the influence of alcohol and DUI—alcohol with a BAC of 0.08 or greater following a traffic stop on Route 41 at Gage Lane. Officers reported Jones was clocked doing 93 mph in a Dodge Challenger and admitted drinking before driving and having a loaded 9mm handgun for which he had no concealed carry permit kept between the driver's seat and center console. Jones later provided a sample indicating his breath alcohol concentration was 0.105.

Adrian Moran, 25, of the 1000 block of Indiana Avenue, Waukegan, was arrested around 11:30 p.m. on Oct. 7 and charged with driving under the influence of alcohol and DUI—alcohol with a BAC of 0.08 or greater following an investigation into a one-car crash on Route 41 near Gage Lake. Officers found Moran had driven a Honda Civic about 30 feet off of the road. Moran said he believed he had lost control due to heavy rain and mechanical issues, but officers suspected he was also under the influence of alcohol. After he showed signs of intoxication on field tests, Moran provided a breath sample indicating his breath alcohol concentration was 0.123.
Kara Bende, 36, of the 2200 block of Telegraph Road, Bannockburn, was arrested around 12:15 a.m. on Oct. 10 and charged with driving under the influence of alcohol after officer pulled over a Land Rover that was driving erratically. Officers reported they smelled alcohol on Bende's breath, and she admitted drinking earlier that night. She refused to provide a breath sample and was also issued an improper lane usage citation.

Antoine D. Cochran, 19, of the 1200 block of Victoria Avenue, North Chicago, was arrested around 2 a.m. on Oct. 16 and charged with driving under the influence of drugs and without a valid license following a traffic stop near the intersection of Route 60 and Ridge Road. Officers reported spotting him make various lane violations and smelling cannabis during a traffic stop. Cochran reportedly told officers he smoked enough marijuana in the vehicle to get high and was taken to Lake Forest Hospital for drug testing.
Sergio D. Carillo, 24, of the 500 block of Great Lakes Drive, Great Lakes, was arrested around 12:45 a.m. on Oct. 23 and charged with battery after officers were dispatched to report of a fight at the Lantern Restaurant, 768 N. Western Ave., where they found a man bleeding from the face and the back of the head. Witnesses were able to identify Carillo as his attacker, and when he was located and questioned, he reportedly admitted punching the man and knocking him to the ground.
David Alvarez, 23, of the 5700 block of West Addison Street, Chicago, was arrested around 2:30 a.m. on Oct. 24 and charged with driving under the influence of alcohol and DUI–alcohol with a BAC of 0.08 or greater after a traffic stop on Route 41. Alvarez admitted drinking tequila before driving and later produced a sample indicating his breath alcohol concentration of 0.168, more than twice the legal limit for intoxication.
Amanda Hulseman, 55, 500 block of Bank Lane, Lake Forest, was arrested around 8:30 p.m. on Oct. 28 and charged with driving under the influence of alcohol and leaving the scene of a traffic crash following an investigation into a two-car crash at the intersection of Western Avenue and Westleigh Road. The 911 caller told police an intoxicated woman had just hit his car and was trying to flee the scene in a silver Mercedes. Officers found a matching car with damage to its passenger side bumper and headlight headed away from the scene on Western Avenue and detained its driver, later identified as Hulseman, who admitted drinking an unknown amount of wine at a bar in Highwood earlier that evening. Police gave no indication she provided a breath test. Hulseman was also issued citations for failure to reduce speed to avoid a crash, driving without a headlight and operating an uninsured motor vehicle.
Lizbeth Flores-Sacramento, 21, of the first block of Prairie Avenue, Highwood, was arrested around 4 a.m. Oct. 30 and charged with driving under the influence, no driver's license and unlawful possession of cannabis following a traffic stop at Westleigh and Green Bay roads. Police reported she remained stopped at a full green light and later appeared disoriented. She reportedly admitted not having a license or insurance and having smoked marijuana and drank alcohol before driving. Flores-Sacramento was also issued a citation for operating an uninsured motor vehicle. Officers found a bag in the car containing cannabis and a grinder.
Dwaine B. Johnson, 37, of the 700 block of South Chestnut Avenue, Park City, was arrested around 3 a.m. on Oct. 31 and charged with driving under the influence of alcohol after officers found him asleep behind the wheel of a car parked on the right shoulder of Route 41 at Deerpath Road. Polcie Police reported Johnson smelled of alcohol and admitted having been drinking. Officers said they found a three-quarters-empty bottle of Don Julio tequila in the back of the front passenger seat.
