Simply tuck your original receipts dated Dec. 1-31 inside an envelope available at Lake Forest retailers and the Chamber office.
3. Enter to win!
When you have $250 worth of receipts tuck them in an envelope with this form and drop it by the Chamber office during business hours (10 am-4 pm, Mon-Fri) or after hours at the mail slot in the door at Pasquesi Home & Gardens, 695 N. Western Ave, Lake Forest (at the train station). After reviewing your receipts chamber staff will shred them or return them to you (please note preference below) and will enter you in the drawing.
RULES: Retail & food purchases only (subject to sales tax). Purchase of autos excluded. May be paid for by cash, credit card or house charge. Only original store receipts dated 12/1-12/31/13 are accepted. Receipts must include name of business and may only be submitted once. Receipts for credit purchases must be store receipts, not swiped credit slips. Entrants must be at least 18 years of age. All entries must be submitted by noon January 5, 2014.
