Little Company of Mary Hospital in Evergreen Park has an opening for palliative care nurse practitioner. The ideal candidate will provide patient care services and act as a program leader and change agent in ensuring the delivery of quality patient care. Assess patient care and the care delivery system and, develop and revise the plan of care, in collaboration with the physician, patient and family and other health care team members and outside agencies. Plan and conduct educational programs. Revise program clinical guidelines/protocols and policies. Perform professional and technical nursing functions for the palliative care program patients. Must possess a current license as a Clinical Nurse Specialist, Advanced Practice Nurse or Nurse Practitioner. Must have current RN license in the State of Illinois. Certification as a Hospice and Palliative Care RN preferred. Five years of experience with three of those years in a CNS/APN role plus two years in palliative care, hospice or related experience.
