Identity Theft

A woman reported to police Nov. 11 that another family member used her identity to visit an eye doctor, according to Palos Hills police reports. Because the family member was uninsured at the time, a collection notice sent the victim a notice for a $150 unpaid bill, report stated.

Break-in

A bag with cell phone parts in it and $100 in cash were stolen Nov. 14 in a break-in at I-Connect Wireless, 10412 S Harlem Ave., according to Palos Hills police reports.

NOV. 15

DUI

Tony Perez, 34, of Downers Grove, was arrested and charged him with driving under the influence, improper lane use, failure to signal and transportation of open alcohol after a traffic stop Nov. 15, according to Palos Hills police reports.

Perez was pulled over at about 1 a.m. after police noticed him hitting the curb several times as he drove southbound in the 7600 block of Southwest Highway, the report stated. During the stop, Perez was unable to complete a field sobriety test because he couldn’t stand on his own, the report added.

Perez’s court date is Dec. 19.

DUI

Ronald Mazeika, 39, of Burbank, was arrested and charged him with felony aggravated DUI, felony driving with a suspended/revoked license, operating an uninsured vehicle, open alcohol and three traffic violations after a traffic stop Nov. 15, according to Palos Hills police reports.

Police noticed Mazeika run through through red light at 111th Street as he was driving northbound on Southwest Highway, the report stated. Mazeika was unable to complete a field sobriety test because he couldn’t stand on his own, the report stated.
