Amazon Faces $28,000 Fine Over Plainfield Warehouse Death
More details of why Amazon is facing a hefty fine from the state following the September workplace death of 59-year-old Phillip Terry.

PLAINFIELD, IN — More than a month after an employee was killed when a forklift fell on him at a Plainfield Amazon warehouse, the company is facing a $28,000 state fine over safety violations, Associated Press reports. According to AP, the Indiana Department of Labor's order says Amazon didn't provide proper training and didn't follow safety procedures before 59-year-old Phillip Terry was crushed to death while performing maintenance work on a forklift's lift. The department's safety order following the Indianapolis man's death gives Amazon until Dec. 1 of this year to correct the safety violations, and until later this month to appeal the fine, AP reports.
Terry suffered several injuries from that accident, with those to his torso and head being most significant.
