This clinic is open to IPS students, staff and families age 16 and older. Anyone under 18 must be accompanied by a parent or guardian.

How to schedule a COVID-19 vaccine appointment:

The COVID-19 vaccine is available at no cost to you. *Your second vaccination dose will be scheduled during your first dose appointment. Prior to your appointment, please visit walmart.com and set up an account. This will allow you to scan a QR code on the day of the event to digitally fill out any paperwork and save time at registration.

PLEASE NOTE:

• Only the Pfizer vaccine will be provided.
There is no cost to receive the vaccine.-Insurance is not required.-This is a 2-dose vaccine. The first shot will be administered on May 1. The second shot will be scheduled for May 22 at the same location.
• An appointment is required.
Call 833-886-0023 to schedule your appointment and select Option 4. Please reference the Indianapolis School District Clinic.
• On the day of your appointment, bring the following:
On the day of your appointment please bring your ID and insurance card (red, white, and blue cardif over 65) if you have one. Please plan to stay on-site for at least 15 minutes after your shot whilewe monitor you for allergic reaction.

*Walmart may bill administration fee to your insurer, health plan or an uninsured fund but will not impose a charge on you.
